Patient Express

Centegra Patient Express vans are available for people who need transportation to or from a Centegra facility. It is a free service for those who do not have a driver’s license, who are being admitted or discharged from the hospital, who are undergoing treatment that prohibits driving or for patients without any other means of transportation.

Where

Patient Express covers all of McHenry County, as well as Fox Lake, Ingleside, Wauconda and Huntley Sun City. Other areas in Northeast Kane and Western Lake counties are considered on an individual basis.

When

Patient Express office hours are Monday through Friday, 7:30am to 4pm. Vans operate from 8am to 3:30pm.

How

When making your appointment, please notify the registrar that you require Patient Express transportation assistance. You will receive a phone call confirming the details of your request within 2-3 days of making the initial appointment. You will receive a second phone call confirming your pick-up time 24 hours prior to your appointment. Your ride is not guaranteed until you receive the second confirmation call. Please give a minimum of 24 hours notice to cancel your ride. You must be available by phone the day of the ride. Patients with walk-in services may call 815-759-3456 to request a ride (TDD 815-385-0285).

Accessibility

  • Wheelchair lifts; 600-lb weight limit
  • Assistance with steps is limited to a maximum of three
  • Caregivers, family or friends may accompany the rider
  • All vehicles are Americans with Disabilities Act (ADA) compliant
  • Seat belts are required
  • Service animals such as guide dogs are permitted
