The heart failure clinic team consists of a board-certified advanced heart failure physician from Loyola University Medical Center, a board-certified nurse practitioner who specializes in cardiology and heart failure and a team of highly educated nurses and clinical staff.
Max Liebo, MD, a Loyola cardiologist and heart failure specialist, will consult on and manage the care of patients with advanced heart failure. Dr. Liebo is board certified in cardiovascular disease and internal medicine. He completed a fellowship at the Cleveland Clinic Foundation in advanced heart failure and transplant cardiology. Dr. Liebo will see patients in the clinic and will direct care to the advanced heart failure specialized team at Loyola if necessary.
Linda Rood, DNP, NP-BC, Centegra’s heart failure nurse practitioner, will see patients in the clinic for routine visits, follow-up visits or when they experience any of the symptoms associated with heart failure. Linda completed her Doctorate in Nursing Practice specializing in heart failure and will be able to adjust medications, prescribe, diagnose and treat heart failure. Linda will communicate with a patient’s cardiologist and/or primary care provider.
The heart failure clinical staff will provide individual education and support, coordinate care and provide telephone support for your family. Our staff also includes a dietician and a pharmacist that will provide free consultations for each patient. These appointments with our dietician and/or pharmacist can be scheduled at the patient’s clinic visit. Cardiac rehabilitation is also a service provided to our heart failure patients that qualify.
