Questions About Appointments
Do I need to bring anything with me to check in at Centegra Physician Care?
Yes. Please bring a photo ID and your insurance card(s).
Do CPC primary care physicians accept my insurance?
Centegra Physician Care accepts most insurance plans. Please bring your insurance card to your visit.
Why do I need to provide a picture ID and my insurance card every time I visit?
Centegra Physician Care needs to verify your personal and insurance information at each visit to protect your identity.
I gave my insurance to a Centegra hospital. Why don’t you have it?
Centegra hospitals have a separate billing system than Centegra Physician Care. In addition, your insurance benefit coverage may be different for hospital services than it is for primary care physician services.

Do I need to pay my co-pay or deductible at the time of service?
Centegra Physician Care requires your co-insurance, co-payment and/or deductible to be paid at the time of service. We accept cash, check, money orders and credit cards.
How do I contact Centegra Physician Care Patient Business Services about my bill?
Centegra Physician Care’s Customer Service line is 815-206-1073. The switchboard is open to take your calls from 8 a.m. to 5 p.m. Monday through Friday.
What are my options for paying my bill?
Centegra Physician Care has made every effort to make paying your bill an easy process. There are three ways to pay:

Will Centegra Physician Care bill my insurance?
Yes. Please bring your insurance card on the day of your service. You will also need to bring any other health forms or insurance forms your insurance company requires such as an accident form, claim form, coordination of benefit form, or a pre-existing condition form.
If I don’t have insurance coverage, will I be seen by a CPC primary care physician?
Centegra Physician Care will not withhold or delay emergency services because of a patient’s inability to pay. Centegra offers a variety of payment alternatives such as interest-free payment plans. A $50 deposit toward the charges for your visit is required. All major credit cards are accepted.
