Physician Office Patient Portal

As of May 1, 2019 and as part of the continuing integration of Centegra into Northwestern Medicine, Centegra Physician Care will officially join Northwestern Medicine Regional Medical Group. All physician practices and Immediate Care Centers will transition to Northwestern Medicine’s electronic health record system on this date. With this change comes a new patient portal, NM MyChart.

Starting May 1, you will be able to open an NM MyChart account. Between May 1 and your next appointment, please call your physician’s office if you would like to request prescription refills or communicate with your care team. Once you have had an appointment, you will be able to request prescription refills, message your care team, pay your physician bill and view visit information in NM MyChart.

Although your patient history will not be transferred to your new NM MyChart account, you and your physician will still be able to view your medical history in the Centegra Physician Care patient portal.

Getting started with MyChart is easy! Just follow these steps:

  1. Visit
  2. Follow the sign-up instructions to activate your account.
  3. Create a username and password, and log in.

You can also set up and access NM MyChart through the MyNM app. To get the app, text MYNM to 43506, or find it in the Apple App Store or on Google Play.
