The mission of the Inpatient Rehabilitation team at Centegra is to guide and enable you to achieve a successful recovery and lead to productive and enjoyable life. Our multidisciplinary team of rehabilitation nurses, social workers, occupational, speech, and physical therapists collaborates with rehabilitation physicians to promote self-care and Activities of Daily Living (ADL).

The Inpatient Rehabilitation Care Center at Centegra Hospital-McHenry offers:

  • 15-bed unit
  • 24-hour nursing care designed to meet your unique needs
  • Semi-private rooms that border the dining area to promote socializing
  • Occupational, speech, and physical therapy six to seven days per week
  • Pet therapy
  • Goal setting to help you regain independence

Your Stay at Inpatient Rehabilitation

Admission Criteria

Medicare closely monitors admission to Inpatient Rehabilitation. The Center for Medicare and Medicaid Services (CMS) has established the following diagnoses necessary for admittance:

  • Stroke recovery
  • Spinal cord injury
  • Congenital deformity
  • Amputation
  • Major multiple trauma
  • Fracture of the femur
  • Brain injury
  • Neurological disorders
  • Active Polyarticular Rheumatoid Arthritis and seronegative arthropathies
  • Systematic vasculitides with joint inflammation
  • Severe or advanced osteoarthritis (osteoarthrosis or degenerative joint disease)
  • Certain categories of hip and joint replacements

Family and Caregiver Involvement

Family and caregiver involvement is critical to your recovery. Support networks help families and caregivers ask questions and learn how to care for you outside of Inpatient Rehabilitation.

Length of Stay

Your length of stay at Inpatient Rehabilitation is determined after the rehab team completes an evaluation and treatment plan. Length of stay depends on the following factors:

  • Ability to participate in therapy
  • Potential to make significant improvement
  • Support system in place upon discharge

Discharge

Centegra’s team of professionals wants to provide the best possible transition for you and your family after discharge. To facilitate this transition, you and your family should:

  • Prepare for discharge when you are admitted
  • Anticipate changes to your home and work pattern
  • Identify support for after care, which includes change to the home environment, daily care, and help with housekeeping
  • Prepare to make alternative arrangements in the event that you do not regain complete independence
