I am delighted to welcome you to Centegra Health System. As the newest member of the region’s leading healthcare system, you are now part of a team of caring, compassionate and highly skilled Associates. During the interview process, you met with many people who will be important to your daily work at Centegra. You were chosen for your new job because Centegra’s Associates believe you will enhance our culture. Your skills, your positive attitude and your commitment to providing high-quality care to our community are what convinced us that you will be a great addition to our Centegra family.
During the next weeks and months you will learn more about Centegra as your fellow Associates learn more about you. As you gain knowledge about our healthcare system and your responsibilities, I encourage you to ask questions and to develop relationships that will form the foundation of your Centegra career. This will be an exciting time filled with new information and challenges as you learn about your department’s processes and procedures. The following forms are to be completed and returned during your pre-employment appointments with Human Resources Development and Occupational Health. Please bring the completed forms at the time of your appointment. Please keep the Human Resources Development and Occupational Health forms separate. Please note that Non-Licensed Associates with direct patient contact must also complete a fingerprinting form during their pre-employment appointment.

- Harassment in the Workplace Policy (please read)
- Policy Signature Form (please sign after reading the above two documents)
- Steps on Accessing HRD Policies and Procedures (please read but note that you will be unable to access Centegra’s policies until your official start date)

Please bring all signed forms along with your Human Resources Development paperwork to your pre-employment paperwork appointment.
Forms to complete and return to Occupational Health:
Please note your appointment with Occupational Health consists of:
- Basic Health Screen
- Titer Test (blood draw for Rubella/Rubeola Immunities)
- Drug Screen (urine test)
- Hepatitis B Information (if required for your position)
- Two step TB skin test
