Where to Go, Who to See
When you arrive at the hospital, enter through the main hospital entrance and the receptionist at the front desk will direct you to a registrar. You will need your:
- Insurance cards
- Your physician’s name and phone number
- Your doctor’s order – this information ensures you receive the correct test or procedure and also helps determine insurance coverage
- Co-payment
- Lists of current medications and allergies
- A bag if you’re staying overnight. Please leave all valuables and jewelry at home.
If you are married, please bring information about your spouse’s insurance coverage. Many people are covered by their spouses’ insurance in addition to their own.
The registrar will require you to sign a consent for treatment. Please read this form carefully before signing it. By signing this form, you give consent for treatment, assign insurance benefits to Centegra and accept financial responsibility for services not covered by your insurance, health plan or another source.
You will be asked if you have an advance healthcare directive such as a Power of Attorney for Healthcare. If you so desire, information about advance healthcare directives will be provided to you.
