Putting the Pieces in Place for You

To be sure you receive all the necessary services during your time as a patient at Centegra Health System, you may be helped by a Care Coordination Team. This group of compassionate and experienced professionals consists of RN case managers, clinical social workers, case management secretaries and physician advisers.

How Will the Care Coordination Team Help Me?

Your Clinical Case Manager will collaborate with you, your family, your doctors and the other members of the healthcare team to ensure the care you receive at Centegra focuses on your needs while you are in the hospital.

The Clinical Case Manager or Clinical Social Worker will assist with identifying and planning for your discharge needs beginning immediately after admission, keeping in mind the goals you need to meet for discharge.  The Clinical Case Manager will communicate with your insurance company as needed.

Our Clinical Social Workers are also available  as resources and support that you or your family might need during your hospitalization. They can refer you to support groups at the time you are being discharged.  We also have social workers available through  Behavioral Health,  Rehabilitation,  Home Health and the Centegra  Sage Cancer Center to provide assistance with resources, counseling and support groups.
