Preparing for Discharge
Your doctor, nurse and other members of the healthcare team will help prepare you to return home from the hospital. The case manager or social worker will help you arrange care or services you may need after you are discharged from the hospital.
Discharge Checklist
- Arrange for someone to pick you up by 11 am on the day you are discharged. The case manager or social worker can help make transportation arrangements, if necessary. It is best to make transportation arrangements the day prior to your discharge.
- Review discharge instructions with your doctor or nurse and ask questions you may have about your medication, daily activities, diet or other concerns.
- Be sure that you understand the instructions for following up with you doctor.
