Records for Outpatient Radiation Therapy and Outpatient Infusion Therapy performed at Centegra Sage Cancer Center can be obtained by calling

All requested records require that a Release of Information form be completed and signed by the patient or Medical Power of Attorney presented (documentation required). A photo ID is required to pick up records.
