Inpatient Rehabilitation

Centegra's multi-disciplinary team of professionals work collaboratively with physicians to promote self-care and Activities of Daily Living (ADL).The mission of the rehabilitation team is to guide and enable patients to successful recoveries that lead to productive and enjoyable lives.

The Inpatient Rehabilitation Care Center offers

  • 15-bed unit
  • 24-hour nursing care designed to meet the unique needs of patients
  • Large, semi-private rooms that border the open recreation area to promote social activities
  • Daily occupational, physical and speech therapy
  • Pet therapy
  • Goal setting to help patients regain independence

Admission Criteria

Admission to Inpatient Rehabilitation is closely monitored by Medicare. The Center for Medicare and Medicaid Services (CMS) has established the following diagnoses necessary for admittance:

  • Stroke
  • Spinal cord injury
  • Congenital deformity
  • Amputation
  • Major multiple trauma
  • Fracture of the femur
  • Brain injury
  • Neurological disorders
  • Burns
  • Active Polyarticular Rheumatoid Arthritis and Serongative Arthropathies
  • Systematic vasulidities with joint inflammation
  • Severe or advanced osteoarthritis (osteoarthrosis or degenerative joint disease)
  • Certain categories of hip and joint replacements

Team Conferences

Team conferences are held weekly to discuss the following:

  • Best treatment plan for each patient
  • Weekly assessments of progress
  • Final recommendations for patient near the time of discharge
  • Post-acute care at Centegra Neurotrauma Day Treatment Center

Family and Caregiver Involvement

Family and caregiver involvement is critical to the recovery of the patient. Support networks help families and caregivers ask questions and learn how to care for the the family member outside of the Centegra facility.

Length of Stay

Length of stay is determined after the team completes an evaluation and treatment plan. Centegra understands that every person rehabilitates at different rates, but the average stay is 10 to 12 days. Length of stay depends on the following factors:

  • Ability to participate in therapy
  • Potential to make significant improvement
  • A support system in place upon discharge

Discharge

Centegra's team of professionals wants to provide the best possible transition for patients and their families after discharge. Patients and families should:

  • Prepare for discharge when patient is admitted
  • Anticipate changes to patient's home and work pattern
  • Identify support for after care, which includes changing the home environment, daily care and help with housekeeping
  • Be prepared to make alternative arrangements in the event that the patient does not regain complete independence.
