At the Centegra Heart Failure Center, our dedicated professionals work to improve your quality of life by managing the symptoms of heart failure. We educate you about the disease and the ways it can be controlled. Our primary objectives are to reduce the number of unnecessary visits to the emergency room, to prevent hospitalizations and to help you live a full and productive life. While participating in the Heart Failure Center program, your physicians will receive continuous updates about your condition.

What is heart failure?

Heart failure is a chronic disease in which the heart’s ability to receive and/or pump blood is impaired so much that the heart can no longer meet the body’s needs. This results in a multitude of symptoms, including shortness of breath, chronic cough, swelling in your legs and or abdomen, congestion of the lungs and increased fatigue.

You Are Not Alone!

Heart failure currently affects 5 million Americans, with more than 500,000 new cases diagnosed per year. It is the most common cause of hospitalization in people over the age of 65. Typically, heart failure is caused by a number of heart conditions including high blood pressure, coronary artery disease, heart valve problems, systemic diseases or congenital heart disease.

Services Provided by Centegra's Heart Failure Team:

  • Provide individual education and support
  • Utilize evidence-based treatments
  • Manage symptoms of Heart Failure
  • Coordinate Centegra's support services (Lab, X-ray, Anti-coagulation Clinic, Diabetes & Wound Clinics, Cardiac/Pulmonary Rehabilitation)
  • Assist with risk factor modification (smoking, high cholesterol, weight management, stress)
  • Assist you by telephone, as needed
  • Provide an intermediary care site between the Physician's office and the acute care institution

As a Heart Failure Center participant, you will be advised to:

  • Weigh yourself and record daily weights
  • Follow a low-sodium diet
  • Limit your fluid intake
  • Take your medications as prescribed, on a regular schedule
  • Employ healthy lifestyle habits (stop smoking, limit or avoid alcohol, eat a heart-healthy diet)
  • Learn to recognize and report increasing signs and symptoms of heart failure and immediately report them to your health care provider
  • Participate in any diagnostic testing as required
  • Cooperate in the management of your condition, which promotes self care

Joining the Centegra Heart Failure Center

If you have been told you have heart failure this program may be beneficial to you. All Heart Failure Center participants must have a physician on staff to qualify for the program. This physician may be your primary care provider, cardiologist or internist.

We encourage patients to have a loved one accompany them to their Heart Failure visits for support and to help in the education process. Loved ones are more than welcome to attend each visit if necessary.

Centegra Health System offers Patient Express as a special service to people who need transportation to the Centegra Heart Failure Center (as well as to other Centegra Health System facilities for inpatient or outpatient care).
