2. Request an appointment online.
3. In person at any one of our locations in Crystal Lake, Huntley, McHenry, Spring Grove or Woodstock

What you will need:

• Insurance Card
• Valid driver's license or ID
• Co-payment

If this is your first visit to a Centegra Primary Care physician, please

• Allow 15 minutes before your appointment to fill out information about your family's medical history
• Have your medical records transferred from your previous physician

If this is your first Obstetrical and Gynecological Appointment, please

• Allow 15 minutes before your appointment time for patient registration
• Have your insurance card readily available along with co-pay amount
  • It is our office policy to require payment in full at the time of your first visit if you do not provide us with your insurance card.
• Print and bring completed registration form
• Print and bring completed OB/GYN health history form
