Case Managers
These dedicated staff members facilitate your admission into our program and work with your insurance company to provide you with the most cost-effective care to meet your needs. We accept reimbursement from Medicare as well as individual insurance plans. A physician’s prescription is required and can be provided by your primary care physician or our medical director.
After Discharge
Our professional team advocates for each patient’s unique goals, which may include returning to employment or school. We offer clinic- and community- based treatment that includes access to our Health Bridge Fitness Centers for aquatics and fitness training.
