Centegra’s Cancer Services provides a caring environment, with convenient hours and a qualified, specialized team of  oncology professionals.

If you need to schedule an initial appointment for yourself or your family member(s) for infusion therapy or radiation treatment, there are several easy ways to arrange an initial appointment or consult.

Infusion Treatment Clinic

Outpatient infusion therapy is used to treat various types of cancers, blood disorders or other conditions in which certain intravenous (IV) medications are prescribed.

Centegra offers two locations for your infusion treatment needs, both staffed with oncology certified nurses.

Chemotherapy, blood transfusions, IVIG and cancer treatment management are the specialized services offered.

Centegra Hospital – Woodstock, Ambulatory Treatment Services (ATS)

A variety of cancer and non-cancer related infusion therapies are provided.

For each location, your doctor must fax a signed order, listed below, along with a current history and physical form.

Radiation Oncology

Radiation therapy is used to treat certain types of cancer as well as non-cancerous conditions.

Using the latest technology, comprehensive radiation therapy services are provided at the Centegra Sage Cancer Center, located at Centegra Hospital-McHenry.

There are two convenient ways to make an appointment.

  • Call (815) 344.8000  to speak with an Intake Coordinator, who will schedule the appointment and answer any questions you may have. This call typically takes 15 minutes.
  • Download the  intake form and fax the completed document to  (815) 344.9601. Within the next business day, a team member will call you to set up your initial consultation.

Consultations are generally scheduled within five business days from your initial call or fax.

FAQs

Do you provide second opinions for radiation therapy?

Yes. We are happy to provide a second opinion. You will need to schedule an appointment and provide basic medical information.

Can I come to Centegra for  chemotherapy if my oncologist is not on staff?

Yes. Physicians who are not on staff must have an order co-signed by an on-staff physician of the same specialty. We can provide the appropriate contact information for the Centegra physician specialty needed.
