Wellness Biometrics/Vitality Check July 2012

At Centegra, we are committed to your health and wellness. An important way to stay well is by knowing your numbers: Height/weight, cholesterol, blood pressure, waist circumference, and glucose. The Wellness Biometrics/Vitality Check is a convenient, confidential, on-site opportunity for Associates to get this baseline screen and identify any potential health risk factors.

When are the screenings in 2012?

We will be expanding the Wellness Biometrics/Vitality Checks to twice a year in July and January. Our next screening is scheduled for July 17 through July 31. Based on your feedback, we have added additional locations for our July screening and will bring the Wellness Biometrics/Vitality Checks to departments throughout the system. Please see the schedule below for dates/times.

Can I get a copy of my lab results?

Lab results will be sent to your home address within 3 weeks of attending the screening. If you have a critical lab value, you will be called by a Wellness Team Member within 24 hours.

What is the price for the screening?

The Wellness Biometrics/Vitality Check is FREE to all Associates ($30 Spouses/Dependents/Volunteers).

Will I earn HumanaVitality Points for Participating?

Participants of the Humana Medical Plan can also earn up to 2,000 points annually for participating in the Wellness Biometrics/Vitality Check.

When will I get my HumanaVitality Points?

Information will be uploaded to the HumanaVitality web site and points will be awarded within 6 weeks of your screening date.

Do I have to make an appointment?

No appointments are necessary, please drop in to any of the dates/times below. You do not need to attend a screening held at the site where you work.

Do I have to fast?

An 8 – 10 hour fast is required for the blood draw, but please drink plenty of water.
